Ambulance Delays severe (2021/22)

Response to this request is long overdue. By law, under all circumstances, South Central Ambulance Service NHS Foundation Trust should have responded by now (details). You can complain by requesting an internal review.

Dear South Central Ambulance Service NHS Foundation Trust,

1. How many serious untoward incidents were recorded by your organisation in 2021/22 where the degree of harm was recorded as “severe” or “death” AND where there was either a delay in the ambulance arriving at the incident scene or a delay in the ambulance handing the patient over to a hospital?
2. Please provide me with a brief summary of each of the incidents in Q.2 which were logged in 2021/22 stating (i) the degree of harm recorded and (ii) a brief summary of the incident including how long the delay was and why the delay occurred and the consequences for the patient of the delay.
