Ambulance delays
Dear East Midlands Ambulance Service NHS Trust,
1. How many serious untoward incidents were recorded by your organisation in (i) 2018/19 (ii) 2019/20 and (iii) 2020/21 where the degree of harm was recorded as “severe” or “death” AND where there was either a delay in the ambulance arriving at the incident scene or a delay in the ambulance handing the patient over to a hospital?
2. Please provide me with a brief summary of each of the incidents in Q.2 which were logged in 2020/21 stating (i) the degree of harm recorded and (ii) a brief summary of the incident including how long the delay was and why the delay occurred and the consequences for the patient of the delay.
