Dear FOI Enquiries,
In February this year I sent a request asking for information about an ambulance that was called to Rhodes business Park in Middleton on 1st August 2016 at about 3pm . I had an answer telling me that this was so but they couldn't give me anymore information without the patients family permission . I am the patients wife Mrs Kathrine Hickey and need to know how my husband was looked after on that day . All I know is that he suffered a fatal heart attack on that day while at work and I didn't find out untill 7:15 pm that evening and it has been troubling me since about what care he was given . I hope you can help me come to terms with this by helping me find out .

Yours sincerely,

K hickey

Dear Mrs Hickey,

Thank you for your email. I am very sorry to read of your husbands passing.

We can give you a copy of the paramedic notes from the day if that would assist you? To do this, we would need a copy of your husband's death certificate, a copy of the grant of probate and a copy of your ID (eg, driving license or passport).

These copies can be emailed or sent in the post to:

Legal Services
Ladybridge Hall
Chorley New Road
Bolton
BL1 5DD.

We look forward to hearing from you.

Kind regards

Adele

Dear Adele Gibbons,

Thank you so much for your quick response to my request about my husband . I received the information today . Although it was difficult to read it has filled in the spaces that enabled me to know that my husband was well looked after by your crew and bystanders but unfortunately nothing could be done to save him. I would like to take this opertunity to thank the paramedic who looked after him on that day if you could possibly send on my thanks and gratitude. Thank you once again North West ambulance

Yours sincerely,

K hickey
