Dear Northern Health and Social Care Trust (Northern Ireland),
I am requesting information as per the following:
Alliance medical currently occupy car parking space at the Mid Ulster Hospital site to provide private medical services to the public and the trust.
Regarding this:
1. Where does this facility hook up to its electric source at.
2. Does it run on the same power lines as the Mid Ulster Hospital. If it is a 3 phase connection where is this connection made.
3. Provide a copy of their public liability insurance to be able to run a facility in a publicly own car park.
4. Provide a copy of any risk assessment carried out by the trust to the location of a private health facility on public.
5. provide a copy of the original tender documents to Alliance medical for this mri service to be located onsite at Mid Ulster Hospital.
6. provide a copy of all current and previous contracts and service user agreements between the trust and Alliance medical for the provision of mri services at the mid ulster hospital site.
7. provide a copy of all payments made to the trust by Alliance medical for the use of public land at mid ulster hospital including water and electric costs if applicable.
