All documentationand full service history in reference to my Prison service 1980 - 2005.

Dear HM Prison Service,
I joined the Service in April/May? 1980. I cannot remember my Service number but my NI number is YE367255D.
I did my training course at Leyhill and was posted initially to Leicester Prison. I transferred to Gartree around 1983 and then to Armley leeds in 1987. In 1993 I was posted to Full Sutton on promotion to Senior Officer. In 1999 I was posted to Wakefield Training College and the posted back to Full Sutton in 2002? on promotion to Principal Officer. I am requesting all documents in regards to my 25 years service.

Specifically I am requesting all my personnel reports, service history and any documentation in reference to any inquiries made into my performance or behaviour either by the Prison service or the Police Force.

This request is generated because I am writing my auto biography and have forgotten so many details. I am aware that there will be a charge for the information.

Subject Access Request

 

Thank you for your email below, in which you asked for information from
the Ministry of Justice (MoJ).

 

Your request has been handled in line with the Data Protection Act (DPA)
as a Subject Access Request (SAR) for your personal information.

 

 1. What information is required by the MoJ to process a SAR request under
the DPA?

 

+ a £10 fee,
+ personal identification and
+ a location for us to conduct a search for your personal information
(for example a particular court or tribunal)

 

 2. What information you need to provide:

 

Please see below for the information required before we can process your
request. The details required will be dependent on your employee status
and if you are a current or ex employee.

 

 

Ex- employees (those who have left the department over 2 years ago):

 

In order for us to process your request we require the following from you:

 

+ A £10 fee

 

The MoJ charges a fee of £10 for this service in line with the provisions
of the DPA. The fee can be paid by either cheque or postal order and
should be made payable to ‘Her Majesty’s Paymaster General’ or ‘HMPG’ –
please do not send cash – this will be returned to you.

 

+ Proof of Identity

 

Proof of identity can be confirmed by providing a copy of the front page
of your wage slip. If you do not have this then please supply a copy of a
recent utility bill and a copy of the photograph page of your passport or
driving licence.  Please do not send originals.

 

+ Correspondence address

 

Please note that we are unable to provide a response to your SAR to a
WhatDoTheyKnow address. Please provide your home or an acceptable e-mail
address.

 

 

3. What Next?

 

Please send all required documentation to the address below. Please note,
if only ID is being supplied this can be scanned and sent via email.
