Alcohol Related Deaths in West Sussex

Dear West Sussex County Council,

I would like to make a request under the Freedom of Information Act as follows:

1) The total number of alcohol related deaths of people who, at the time of death, were in treatment in West Sussex in services commissioned by the DAAT (Addaction, NHS and CRI) for each calendar year from 2004 to 2015.

2) The total number of alcohol related deaths of people who in the three months prior to the date of death were discharged from treatment (and had not been subsequently taken back in to treatment) in West Sussex in services commissioned by the DAAT (Addaction, NHS and CRI) for each calendar year from 2004 to 2015.

3) The total number of alcohol related deaths of people who in the six months prior to the date of death were discharged from treatment (and had not been subsequently taken back in to treatment) in West Sussex in services commissioned by the DAAT (Addaction, NHS and CRI) for each calendar year from 2004 to 2015.
