Please provide copies of:

1) Any contracts, memoranda of understanding, or other agreements (especially data sharing agreements and data contracts) between the RNOH and the company;

2) Any statements of conflict of interest, or other discussions, shared with the RNOH Board about that company by any of the company's registered officers (see above link);

3) Any Information Governance documents, Data Protection Impact Assessments (or pre-GDPR Privacy Impact Assessments) created by the RNOH or the Company about projects involving both, and including any projects that involve other entities;

4) Whether the RNOH has been formally informed of any valuation of the company, and, if so, for the basis of that valuation;

5) Whether any agreement allows an officer of the company to use their RNOH / NHS credentials to create a dataset for the company; b) any sanctions should they have done so; and c) any assurances on provenance of the datasets that orth.ai are using in any work with the RNOH.

6) How many records RNOH believes the company has acquired from databases and clinical systems in use at the RNOH.

Please do not delay your response on the basis that you cannot provide some or any of the information for either one of the items above; I am happy for you to provide partial information in a timely manner, on the understanding that you will provide more complete information as soon as it is available.
