Nottingham University Hospitals NHS Trust (NUH) came into being on 1st
April 2006 and assumed responsibility for the services provided by both
the Queen's Medical Centre (QMC) and Nottingham City Hospital NHS Trust
(NCH).
Your request for information has now been considered and I am pleased to
be able to answer your questions as follows:

We can confirm that the Trust does not hold the information falling within
the description specified in question 3.
However, having reviewed your request for information, we have identified
that, due to the nature of the information that you are seeking, your
