Dear 44 Dental Care,

This request is under the Freedom of Information Act/ Environmental Information Regulations.

Could you please supply me with data from the last 5 years on children's dental health. Including specialist referral statistics and administered fillings/tooth decay prevention as well as the ages of these children during this time period.

Please include copies of material which you hold in the form of paper and electronic records, including emails.

I would be grateful if you supplied me with the information within the next 20 days as the law permits.
