Dear Oxford University Hospitals NHS Trust,

For patients with a Buckinghamshire postcode e.g. SL7, HP12 and Oxfordshire postcode, please provide the number of A&E attendances to Oxford's John Radcliffe hospital for the following dates:
1) 1st April 2017 - 26th March 2018
2) 1st April 2016 - 26th March 2017
3) 1st April 2015 - 26th March 2016

Of these patients and with the same dates above, please provide the number of patients who were a) admitted and b)readmitted to hospital within 30 days

Please provide the data broken down by postcode e.g. SL1, SL2 and in a PDF format.

University Hospitals NHS Foundation Trust can confirm that it holds the

In order to comply with our obligations under the Data Protection Act we

are less than 5.  This is to reduce the possibility of any individual
being identifiable from this information or by combining it with
