Please provide me with the following information within section 12 of the Freedom of Information Act

1. Total number of ADR reports in association with a vaccine between 01/01/2005 and todays date
2. Total number of ADR reports in association with a vaccine that are considered serious and those that
are considered non-serious between 01/01/2005 and todays date
3. Total number of ADR reports in association with a vaccine between 01/01/2005 and todays date in
children under the age of 10, between 10 and 18, and in adults 19+.
