Dear Greater Glasgow NHS Board,
Could you please provide the following information:

1) How many adverse events were recorded across NHSGGC in the years 2013, 2014 and 2015. Of these, how many involved patients with a learning disability or autism.

2) How many adverse events involving adult patients were recorded, broken down by department, at the Royal Alexandria Hospital in the years 2013, 2014 and 2015. Of these, how many involved patients with a learning disability or autism.

3) How many adverse events involving adult patients were recorded, broken down by department, at the Queen Elizabeth University Hospital for the most recent year(s) you have data. Of these, how many involved patients with a learning disability or autism.

4) If adult patients with a learning disability or autism are having difficulty accessing hospital care to meet their needs, or are unable to access hospital care to meet their needs what procedures do NHSGGC have in place to assist them? What office/person do patients contact to access this assistance?

5) If adult patients with a learning disability or autism are having difficulty accessing primary care to meet their needs, or are unable to access primary care to meet their needs what procedures do NHSGGC have in place to assist them? What office/person do patients contact to access this assistance?

Further to our acknowledgment of 27 October 2017, we have made enquiries
within NHSGGC in order to locate the information requested by you, and
this has generated some queries that require clarification before we are
able to continue with your request.

 

In relation to Questions 1, 2 and 3, ‘adverse events’ recorded on our
incident reporting system could cover a wide range of occurrences, from
minor events (for example slips/trips and falls) to significant adverse
events categorised as severe/major.  To assist you, incidents are rated
from 1 (minor or negligible) to 5 being the most severe.  Reporting on all
adverse events may return a large amount of data.  Can you please clarify
whether you wish us to report on all adverse events, or only the more
severe events (eg 4 and 5)?

I have also been advised that our incident reporting system would not
specifically identify whether a patient had a learning disability or
autism.  However, it may be possible to provide information based on
location such as ward areas where learning disability patients are treated
or accommodated.  Can you please clarify whether you would wish us to
proceed to try and provide data on this basis. 

 

I look forward to hearing from you further, but would ask you to note that
we are unable to progress your request until we receive this clarification
from you.

Dear FOI BOARD HQ,

As you do not hold data regarding adverse events involving patients with autism or a learning disability you cannot answer my original request in full.

Please provide the information in the remainder of the request. Especially how people with a learning disability or autism are to receive equality of access to NHS care in NHSGGC.

In addition please provide the following:

Regarding adverse events of level 3, 4 and 5: do you hold data of the number of adverse events involving those with protected characteristics under the Equality Act (i.e. age, sex, race, disability)?

How does NHSGGC ensure people with autism or learning disabilities are not disproportionately represented in adverse events of level 3, 4 or 5 if no data is collected?
