Further to my email of last week, the team involved in collating the
information you have requested, have asked me to write to you. Your third
question asks about the structure of the placement team.  We need some
more information on what ‘placements’ you mean. Your fourth question asks
about details of discharge teams and where they are based. Again, we need
more clarification of on the meaning of ‘discharge’, is this linked to or
from residential or hospital care?

East Sussex County Council’s department for Adult Social Care makes use of
a number of professional tools to assist in the calculation of care home
fees. These are used to determine how an individual’s needs can best be
met while considering equality and fairness with value for me and the cost
to the public purse. The tools in use are as follows:

* East Sussex Published Rates. Generally used for placements for
Older People in residential or nursing home placement, although
they may apply to adults with similar assessed needs.
* Directly Provided Services Rates. An indicative price list for care
and support at home or in the community.
* Care Funding Calculator. Most often used for adults or older people
with specialist needs as a benchmarking tool. This tends to be for
working aged adults with complex health needs.

The rates are structured as follows: 

Standard rates residential care (only);

Enhanced Rates *– nursing care ;

Standard residential care with nursing care;  

Enhanced rates* to reflect - Dementia nursing care and
other specialist needs such as  

·       Physical Disability

·       Mental Health

·       Learning Disability

* The enhanced rates reflect the higher level of specialist support that
is essential to meet the eligible social care needs of the client.

All funding decisions are based on an individual’s circumstances and are
needs led. When assessing people’s needs under the NHS & Community Care
Act 1990, the Council is entitled to take into account its own resources
as well as the clients preferred choice when deciding how best to meet
someone’s social care needs.

It is mainly, though not exclusively, workers within the Neighbourhood
Support Teams and the Hospital Teams who assist older people to find
residential placements. The attached chart shows these teams.

We publish one contact number for all teams which is our contact centre,
Social Care Direct, on 0345 60 80 191.  This can be used by someone who
has not been in contact with us before and also by clients who want to
contact their named worker.

4) Details of discharge teams and where they are based. Discharge team
refers to the teams based at hospital or at adult social services (older
people) who coordinate the discharge of service users to their own homes,
care homes etc       
The chart gives information on the main teams that are involved in
hospital discharge and their main base (marked in red).
