Please provide a full blank copy (ideally pdf) of your contact, referral, social care assessment, support plan, review, and panel application forms as per your adult social care records system.
Please state which electronic records system you are using in your adult social care teams (where there is more than one system used by social workers supporting adults please list all).
Please state when this system was first rolled out to adult social care teams.
Please explain, where possible, how your documentation has been tailored to be care act compliant.
This request will be actively monitored including regarding the timeframes and comprehensiveness of the response. Please be aware that delays and incomplete replies will be escalated as per FOI procedures.

Dear Mark Jones,

4. All our documentation was reviewed in line with the Care Act
requirements, forms were changed to ensure they use Care Act language and
eligibility criteria.
