Dear Liverpool City Council,

Adult Social Care Survey 2012

1. Why does the survey have a number on it?
2. Who is the survey aimed at? For example is the survey aimed at:-
people in care homes,
people receiving care at home,
people using day centres.
people in supported living
3. If support/agency staff complete the form for those using their services, how much will Adult Social Care take into account the answers (which could be for the benefit of the agency).
4. What is the purpose of the survey.
5. What will Adult Social Care do with the information received from the survey.
6. Will the results of the survey be published.
7. Is the survey connected with the Community Care Assessment/Care Plan.

Thank you for your recent correspondence requesting information from the
City Council.  Your request was received in our offices on 22 March 2012

If the above summary is incorrect or you wish to change any details, let
me know at the earliest opportunity.

A response will be sent to you as soon as possible and in any case within
the statutory deadline which is 20 working days from the working day after
your request was received.

For your information, the Act defines a number of exemptions which may
prevent the release of information you have requested. There will be an
assessment and, if any of the exemption categories apply, the information
will not be provided to you.  You will be informed if this is the case
