Dear Mr Read,

Thank you for your request for information. We require clarification from you in order to proceed with your request.

Please can you clarify your request for information for question 1. We are unsure what you mean by how many reviews/reassessments have not been undertaken within a 12 months and those that fall outside of care act compliance? Are you requesting how many adults were without a support plan after a review/reassessment? We would not be able to accurately report this information as some customers do not have support plans uploaded onto our system, therefore our records do not accurately reflect those with or without a plan.

We would also advise that the Care Act does not define what constitutes a timely review, therefore with this, and the information mentioned above, you may wish to revise your request.

Please can you provide the requested clarification within 3 weeks of today. Please quote the reference number provided above.

Once you have clarified your request, we will be able to process it. However, if you do not provide the requested clarification within the specified timeframe your request will lapse and we will close the case on our system.

We look forward to hearing from you.

Yours sincerely,

Information Governance team

Dear Newham Borough Council,

Thank you for the information provided. Can i just clarify that your response to Q4 relates to the number (2.306) of reviews/reassessments that were conducted by Sanctuary on behalf of the council and not the total number conducted by the council?

Yours faithfully,

Mr P T Read

Newham Borough Council

Dear Mr Read,

Thank you for your email.

Of the 2,306 reviews undertaken Sanctuary conducted approx. 300 of these were completed by the London Borough of Newham.

I trust this responds to your enquiry in full.

Yours sincerely,
