Our response is as follows:

“Please provide a copy of all Questions that a Social/transitions Worker will use during an Adult Social Care/ transitions assessment to ascertain a care package & obtain an indicative budget.
Please provide a copy of the RAS used in determining said budget.”

Please see attached. All transitions cases are subject to an assessment and a checklist is completed to ensure that all essential information is presented to a panel.

Once the case is allocated to the relevant team, the allocated worker undertakes a full Care Act assessment and the Ready Reckoner is used to determine the indicative/ personal budget.
