Admitted Failures: Reasons and Remedies

Response to this request is long overdue. By law, under all circumstances, Cambridge University Hospitals NHS Foundation Trust should have responded by now (details). You can complain by requesting an internal review.

Dear Cambridge University Hospitals NHS Foundation Trust,

A Trust letter of August 2nd 2013 (ref: X0164/13) apologised that an ophthalmology appointment had not been been made and provided as prescribed, and stated that:

1. "The Trust is sorry that the delay and the reasons for it were not communicated.....".

In respect of these admitted failures, please will you provide the following information:

1.1 The outcome of the Trust's investigations into why the delay, and the reasons for it, were not communicated.

1.2 How the Trust is ensuring that similar failures in administration and communication do not occur again.

The letter also states that:

2. "The Trust is sorry that a suitable new appointment was only offered to you when you made contact with the department ......"

In respect of this admitted failure, please will you provide the following information;

2.1 The outcome of the Trust's investigation into why a suitable new appointment was not offered until the department was contacted.

2.2 How the Trust is ensuring that a similar failure in communication does not occur again.

Dear Mr Rudkin

You will receive a response to your request by the end of the week. I once again apologise for the delay in responding to you within the timeframe we had previously stated. The delay in responding to you has been due to a variety of reasons including staff changes, staff absences and other pressures on staff time but this in no way excuses the fact that we have not complied within the timeframe set out in the Act.
