Dear Dr Ali Hansford

Re: Freedom of Information Act 2000 – Acknowledgement

Further to your request.  The Trust cannot complete survey monkey requests
under the requirements of the Freedom of Information Act.  The survey
monkey approach does not allow us to follow our protocols, approval
processes and codes of practice.

We are very happy to respond to a request in an Excel spreadsheet or Word
document.  Whichever you prefer, we need the freedom to respond to each
question in such a way as best fits the appropriate response from our
services.

I am sorry for the inconvenience caused.

Yours sincerely

S Edmunds
