Dear Imperial College Healthcare NHS Trust,

In order for you to see all the questions in the request on one page, please find them here: https://docs.google.com/document/d/1lHjY....

If the answers could still be provided online via the survey link here (https://www.surveymonkey.co.uk/r/AS_pros...) that would be very much appreciated.

Dear Dr Ali Hansford,

Thank you for your Freedom of Information request. I am unfortunately
unable to open google docs as this is blocked by the Trust’s firewall. Is
it possible to be emailed the questions, please?

Once we have received these, we will review the request and investigate
whether there is any information available for disclosure. In accordance
with Section 10 of the Freedom of Information Act 2000, Imperial College
Healthcare NHS Trust will reply to your request promptly and in any event
no later than the twentieth working day following the date of receipt.

The Trust’s response to your request will contain information available
for disclosure or explain in detail the reasons why we may have been
unable to supply all of the information you requested. It may also detail
where you can find information that is already publicly available or if
the Trust does not hold the information, where you will be able to access
it. It will also detail your right of appeal if you feel that the Trust
has not treated your request fairly.

Kind regards,

Barney

Dear Barney,

Apologies. Please can you try this link instead to see the questions? https://tinyurl.com/y9o9syqu

And the survey for replying is here: https://tinyurl.com/yarxbu3e

I think the links were getting compressed before and not working properly.
Otherwise, I can email the questions to you.

Many thanks,
Ali

Dear Ali,

Apologies, I’m still unable to view the questions via that link. I would
be grateful if you could email the questions to me.

We will try and respond via the survey (the link for this does work), I
just need to see the questions first to know who would be best placed to
answer for the Trust.

Thank you,

Barney
