Dear Dr Hansford

 

I am writing in response to your e-mail below requesting information from
Cardiff and Vale University Health Board (the UHB) under the Freedom of
Information Act 2000.

The Act gives individuals the right to access official information. It
allows the right to request any recorded information held by a public
authority, such as the UHB.  A request can be made for any information it
is thought a public authority may hold. The right only covers recorded
information which includes information held on computers, in emails and in
printed or handwritten documents as well as images, video and audio
recordings.

 

A request can take the form of a question, rather than a request for
specific documents, but an authority does not have to answer your question
if this would mean creating new information or giving an opinion or
judgment that is not already recorded. The information you are seeking
consists of a series of questions that appear to constitute a
questionnaire and whilst the UHB may hold some of this information, as
indicated above, responding would require the UHB to create new
information for the purposes of response therefore we will not deal with
this under the Freedom of Information Act 2000.

To comply with our obligation contained within section 16 of the Freedom
of Information Act 2000, the duty to provide advice and assistance, I
would like to direct you to our web site where information on GP's can be
found. The reason for directing to primary care is that they arrange the
testing for blood monitoring of PSA levels which monitors for this
