Thank you for your further email regarding your request for us to provide
information via a link to Survey Monkey. 

Unfortunately the link that you provided to the individual questions is
broken, and we are therefore unable to view these.

 

One of the requirements of FOISA, in order for a request to be valid, is
that a request must be:

"In writing or in another form which, by reason of its having some
permanency, is capable of being used for subsequent reference (as, for
example, a recording made on audio or video tape)" (Section 8(1)(a))

 

The Survey Monkey process requires that we must provide a response to each
question in order to move to the next screen.  We are therefore unable to
appropriately view the questions to allow us to provide these to the
relevant service.  In addition, I am doubtful whether the survey monkey
process would allow us to attach appropriate documents, or to provide a
formal response under FOI legislation which is in compliance with the
requirements of FOISA and the relevant Section 60 Code of Practice.

While I appreciate that we must provide appropriate advice and assistance
to applicants who wish to request information from us, on this occasion we
are unable to proceed with your request in its current format.

 

I would be very grateful if you could therefore please provide a word
document that contains the requests for which you seek recorded
information held by NHS Greater Glasgow and Clyde, following which we will
provide a formal response and, if it is possible to do so, we will also
complete the survey monkey.

We are treating your request under our procedures for responding to
requests for information under the Freedom of Information (Scotland) Act

I can confirm that NHS Greater Glasgow & Clyde (NHSGGC) holds information
as described by you and our response to your request is attached above. 
We have converted your survey money questions into a word format in which
we could source the information from.  The answers are in bold.

 

I hope this information is helpful.  If you are not satisfied with our
response to your request, you have a right to request a review of this
decision within 40 working days of receiving this response.  The procedure
for consideration of a review is detailed in the attached note.  The note
also describes your right to pursue the matter with the Scottish
