Under the Freedom of Information Act, I hereby request the following information:

A full disclosure of all cases of patient deaths and injury registered within all NHS Trusts & NHS funded care providers from the years 2018-2020 as a result of the ingestion of non food items. To include Hospitals, Care Homes, Hospices and Mental Health Units, Ambulance Trusts, Community Nursing and Community Therapy Units.

I do not wish to know any individual patient details, but a disclosure of what was ingested, where and when within the NHS this occurred.

I request that for each case of death or injury as a result of ingestion of a non food item, confirmation of what action the National Patient Safety Alerting Committee took in each case. Was an alert raised in each case or not, what was the alert, was the ingested non food item restricted in supply across the NHS?

NHS England may hold information relevant to your request.

However, we consider that to respond to your request as it is currently
framed would exceed the cost limit as set out in Section 12(1) of the FOI
Act. Section 12(1) states that a public authority can refuse a request if
complying with it would exceed the appropriate limit of £450 (which
represents 18 hours).  This represents the estimated cost of one person
spending this time in determining whether the information is held, and
locating, retrieving and extracting the information. We have outlined
reasoning for invoking Section 12(1) below.

To locate and retrieve the information you have requested would require
NHS England to review records from the Strategic Executive Information
System (StEIS) to establish which records are relevant, followed by expert
clinical review of open text fields with incident descriptions to
establish whether cases involved ingestion of non-food items (since this
is not recorded as a standard incident category).  For the purpose of this
request an assessment was made of the number of entries on StEIS that
might contain information relevant to your question.  StEIS contains
records numbering in the tens of thousands spanning several years, and we
have filtered on the incident descriptions to check how many may be
relevant using search terms related to your query.

In relation to the most recent two years of StEIS data available, there
are over 100 records relating to ingestion which may or may not involve
non-food items which would require locating and reviewing in order to
extract relevant information in relation to your request. We have
estimated under section 12(1) of the FOI Act that to locate, retrieve,
extract and collate the information requested is likely to take at least
20 minutes per record to conduct and therefore, take up to and over 33
hours to complete. As a result, this takes over the time and cost
threshold in considering an FOI request and any appropriate action to be
taken.

Section 12(1) is an absolute exemption therefore we are not required to
assess the public interest test in its application.

However, if you were to refine your request for information within more
specific margins, for example; giving specific search parameters or
requesting data over a shorter time period, then we may be able to
continue processing your request. Please note we cannot guarantee that
Section 12(1) or any other exemptions will not apply to any information
requested.

We hope this information is helpful. However, if you are dissatisfied, you
have the right to ask for an internal review. This should be requested in
writing within two months of the date of this letter. Your correspondence
should be labelled “Internal Review” and should outline your concerns
