1.  Is there a shortage of supported living accommodation for young people aged 16 
to 24 years? 
 
No, not in Worcestershire.  
 
2.  How does a new provider for supported living accommodation receive referrals 
from the council? 
 
They would need to sign up to the Placements Portal 
 
3.  What are the contact details for the persons responsible for managing the 
supported living framework for young people? (please include telephone number, 
email address, full name and physical office address)

4.  What is the process of referrals for young people in supported living 
accommodation? 
 
Social Worker's submit an Outreach Referral Form following discussions 
with their Team Manager and Group Manager.  They risk assess and 
discuss matching with Outreach team.  If placement is not offered 
internally, permission is sought from the Service Manager to search 
externally. 

 
5.  What are the contact details to request more information on how supported living 
works in Evesham and Pershore?

No 
 
7.  May you give me a list of providers or organisations who provide support for 
young people in Pershore, Evesham and surrounding areas? 
 
We have one provider that provides supported lodging placements in this 
area - St Basils. 

 
8.  How are the costs for supported living accommodation paid? 
 
Providers are paid 28 days in arrears for support hours and 
accommodation. 

 
9.  How is supported living for young people assessed and please provide contact 
information for the department that makes the assessment. 
 
Risk assessments, referral forms and social work assessments 
 
10. How can a new provider be included on the framework for supported living for 
young people or for spot contracts? 
 
Spot providers can sign up to the Placements Portal online:
