This is the response following the internal review of your request for information
under the Freedom of Information Act 2000 (the Act).
I am writing in response to your email, received on 03 March 2016, asking for Hampshire
Hospitals NHS Foundation Trust (the Trust, HHFT) to conduct an internal review of your
request for information dated 29 January 2016.
The internal review process requires that your initial request is reviewed and a new
consideration is made based on all the available evidence. The internal review process does
not seek to review the original decision made by the Trust.
I have undertaken this review on behalf of the Trust as an individual who was not involved
in the decision making of your original request.
Request
Your initial request for information made to the Trust on 29 January 2016, asked for
information on the Hampshire Health Record (HHR), specifically the process when someone
dissents to their information being used.
In response to your request, the Trust provided you with a link to the HHR website where
further information can be found on the process, including the opt-out form.
Internal Review
You have requested an internal review and have clarified that you would like to know
whether the Trust has a process for patient’s to dissent to their information being provided
to the HHR and whether the Trust has an opt-out form.
In response to your questions, the Trust does not hold this information and does not have
its own opt-out form. Patients who do not wish for their information to be provided opt out
through their GP and the HHR website; this is not done through the Trust directly. People
who choose to opt out are checked and removed as part of a filtering process whenever
data is uploaded to the record.

The process for uploading data from the Trust is as followed:
The Trust submits data to the HHR through a secure upload
The HHR filters data contained within the file
During this filtering process, information that matches certain criteria – including
whether the person has opted out – is rejected
The rejected data within the file is deleted
This completes the response to your request.
In your request for an internal review, you stated: ‘…
HHR or any other IT programme that
Hampshire Hospitals NHS Foundation Trust is responsible for.’ It may be helpful to clarify
that HHFT is not responsible for the HHR; this is the NHS South, Central and West Clinical
Commissioning Support Unit. It may also be helpful to know that HHFT only uploads data for
the purposes of the health record; the Trust has opted out of making our data available for
secondary uses such as the analytics database which is used for research.
