Dear Dr Bhatia,
Your request for information under the Freedom of Information Act 2000
I am writing further to your request for information regarding dissent from processing personal
confidential data to the HHR.
I can confirm the following in response to your request:
1. Is your trust one of a number of that process the personal confidential data of all patients
treated/investigated by your hospitals and upload it to the Hampshire Health Record (HHR)?
Yes, we upload all discharge summaries to HHR automatically on discharge.
2. I am interested in the process by which patients may express their objection to such
processing and prevent the uploading of their data to the HHR (whether they have opted out
of the HHR at their GP surgery or not, or whether they are a local resident or visiting from
outside of the area), and whether this has changed in any way recently?
This is the responsibility of HHR, HHR only process those discharge summaries for which they
have patient details.
3. Please could you provide me with the process by which patients may express their objection
to such processing and prevent the uploading of their personal confidential data to the HHR,
pursuant to the Data Protection Act and the NHS Constitution. If patients have to fill out a
specific opt-out form, please provide this to me, together with the details of what ID to
include, who to send the form to etc.

*Please note this process applied across Andover, Basingstoke and Winchester.
This letter confirms the completion of this request. A log of this request will be held on a database
held by the Trust.

If you are unhappy about the response you have received your first line of action should be to write
to request that the Trust undertake an internal review of your application. A senior member of staff,
who was not involved with your initial application, will undertake this review. If after this process
you are still not satisfied with the response you receive from the Trust you can complain to the
