Dear Wiltshire Council,
I have just finished reading a report titled " Independent Investigation Report into the Alleged Falsification and/or Alteration of Service User Records - Avon and Wiltshire Mental Health Partnership NHS Trust ", after NHS South Gloucestershire, NHS Wiltshire, WILTSHIRE COUNCIL , and NHS South of England ordered a report following 30 Social Workers employed by WILTSHIRE COUNCIL Whistle Blew that they had been INSTRUCTED to Falsify or Alter Service User Records of mentally ill patients under the duty of care of Avon & Wiltshire Mental Health Partnership. The report appears on the Whatdotheyknow website, under Avon & Wiltshire Mental Health Partnership - The 'Wiltshire Report ' despite all the denials and spin from AWP it DOES exist after all ! dated 15 09 13, attached to their 'response letter'. I write to ask to see under FOI - 1. The formal letter of agreement WC has with AWP - rather than a full section 75 agreement . 2. I would like to be provided with a copy of Wiltshire Councils own report ( Initial Investigation ) identified as " A subsequent Wiltshire Council led investigation carried out in March 2012 clarified that nearly 30 social workers working in either adult community or liason and later life services shared the same concerns " It was found " 17 patient records were reviewed and cross referenced and exposed a lack of consistency, ommissions and false reporting. The records contained events that did not happen .... " it doesnt get much better ? For Wiltshire Council - George O'Neill - Head of Service Mental Health was Lead Co ordinator ( the man who FLATLY REFUSES to allow my having any contact with him ), and will certainly be able to assist you in finding the information I am seeking. 3. I would also like to be provided with the ' Wiltshire Council AWP Safeguarding Update Undated ' mentioned in the reports appendix. 4. The email from George O'Neill to David Bolwell re Wiltshire Council Concerns dated 15th Feb 2012. 5. Briefing on Avon & Wiltshire Partnership NHS Trust - Strictly Confidential George O'Neill 17th Feb 2012 6. AWP investigation into safeguarding concerns of 15 service users (undated ). 7. AWP Supervision of staff in relation to Safeguarding Report 11 05 12 . 8. AWP Email dated 08 March 2012 re Face to face Contact JC (client) 9. Letter dated 30th April 2012 from Mary Monnington ( NHS Wiltshire ) to Paul Miller ( AWP ) re: notification of an independent investigation . ( AMPLE TIME TO COVER THINGS UP THEN ? !! )
If you hold other documents in respect of this investigation I ask you to also provide them to me please. I am especially interested to see how Wiltshire Council and AWP decided to inform and support patients of the trust whose records may have been 'affected' , in light of Wiltshire Councils strap line ' Where Everybody Matters ' and the duty of care and many Statutory duties and responsibilities you have, especially to these ' at risk patients ', and of course in light of Francis and AWPs duty of candour and putting the patient first in all we do ! YOU MATTER - WE CARE is the latest AWP ' Spin ', as a current patient of AWP who has repeatedly complained to them my records have been falsified and they are not complying with the Strict requirements of the DPA or the handling of Sensitive Personal Data, or the many other requirements to keep up to date accurate medical records, they are now further delaying my treatment, will not allow me to see or have an up to date copy of my records and data, and simply wont respond to my recent letters, are taking away my RIGHTS - SUBJECTING ME TO ABUSE TOO ! Please respond to this FOI request to this website please, and ALSO send me paper copies please.

Thank you for the acknowledgement you sent on 24 10 13 - it was appreciated. However, a note placed on my request by 'whatdotheyknow' states ' By Law, Wiltshire Council should normally have responded promptly and by 21st November 2013 ', and Wiltshire Councils acknowledgement states " you will receive the information requested within the Statutory timescale of 20 working days of receipt of your request ".
As last Thursday 21st was the 20th working day, and today is Monday 25th - I regret to note Wiltshire Council does not appear to have complied with the Law or its Statutory Duties to me ? WHEN you are able to provide your full response to the whatdotheyknow website here please, would you also be kind enough to post me paper copies also please ?

Thank you for acknowledging my FOI request on 24 10 13, and I have since contacted you again on 25 November as no response or update had been provided within the Statutory timescale for you to respond - that date was 21 11 2013.
Regretably my contact with you on 25 November has been ignored - other than an automated ' out of office ' response sent on 25 11 13 indicating Anna Browne would be back in the office on Tuesday 26 11 13.
