I would be grateful if you could provide the following information
- Discharge checklist if used in your trust
- Discharge policy or procedure that is followed in your trust
- Procedure followed for the discharge of anticoagulation patients
on warfarin
-Process map followed within the division of medicine from patient admission, transfer to specialty ward, decision to discharge and discharge
- Measures taken by your trust in order to implement a productive
ward including ward round (and associated documentation used to the
effect)
-Tools or systems your trusts uses to monitor patient flow and discharge
-Measures in place in your trust to ensure patient discharges are planned and they leave the hospital by 12pm where possible to aid patient flow.

Our response:

4. The Trust is in the early stages of implementing a “productive ward
including ward round”. There are currently no documents relevant to your
request at this time.
5. The Trust currently uses an electronic white board that tracks patient
discharge planning (i.e. seen by MDT, TTOS, etc.). This board is
monitored throughout the day and discussed at four ‘bed meetings’ each
day.
