Thank you for your request for information, which we received on 11th June 2013. You
requested information regarding a news article published on the News Shopper website in
relation the incident that took place at River House, Bethlem Royal Hospital on 1 October
In our previous response dated 20th March 2013 we informed you that the investigation had
not been completed as it was still ongoing.
The independent investigation of the incident that took place at River House, Bethlem Royal
Hospital has been completed. The Trust is aiming to publish the report by the end of July on
the website at www.slam.nhs.uk.

The information supplied to you continues to be protected by the Copyright, Designs and
Patents Act 1988. You are free to use it for your own purposes, including any non-commercial
