Freedom of Information request received on 20/04/2018.

FOI Ref: 1462-18

You asked:

Please provide a copy of all whistleblowing reports received by the board
of the Trust (or the board of predecessor organisations) in the last 5
years.

Please redact where necessary those sections of a report (redacting where

this request) that the information remain confidential).

When redacting any section of a report, please provide a full statement of
the reason for the redaction and confirmation that the public interest has
been applied (where the law requires this).

In response:

Please find attached the response from the Trust.

Where information has been redacted it is to protect the identity of staff

director level and therefore this information is exempt under section 40
(personal information) of the Freedom of Information Act 2000 , as the
information constitutes third party data. Section 40(2). Under the FOI Act
disclosure of this information would breach the fair processing principle
contained in the Data Protection Act 1998 (DPA), where it would be unfair
to that person and is confidential.
