UKBA losing documents

Dear UK Border Agency,

Recently I have had several clients who's applications were submitted to yourselves where the UKBA has lost essential documents such as payslips, passports, employment contracts, bank statements and/or sent our client's documents out to the wrong people mixed in with the documents from their own applications.

Upon contacting our client's respective local MP's we were told numerous times across several different constituencies that this is "A common problem that is becoming more widespread"

As this constitutes a serious breach in data protection and in the interests of public awareness I would like to know the following:

Between January 1 2012 and December 31 2012 how many complaints did the UKBA receive in regards to lost documents that applicants had submitted?

Between January 1 2012 and 31 December 2012 how many complaints did the UKBA receive in regards to applicant's documents being sent to the wrong people?

Of the above complaints how many reached a resolution whereby the documents were located and successfully returned to the client?

I would also like to know the answers in relation to the 3 questions above pertaining to complaints made between January 1 2013 and March 1 2013.

I would also like to know how many instances there has been of applications refused due to lack of documentation where the applicant has asserted those documents were provided between January 1 2012 and December 31 2012 whereby the decision for refusal was reversed on appeal.

Thank you for providing the answer to question 1. The fact that the UKBA does not have a system in place that verifies success in lost document cases is either truly abysmal or there is a deliberate lack of transparency here. I am also at a loss to understand how question 1 could be answered but not the subsequent question pertaining to more recent events. I would like to know where this information will be published and when it was decided that this would happen as I hold serious doubts regarding this due to the length of time it took for my request to be conducted. Logically, question 1 should have been declined as well if this is something the UKBA is transparent about as it should have already been published previously.
