Dear Department for Work and Pensions,

I am sorry but I do not accept your response. There are numerous forms available for when a change is reported eg information about a second property the claimant owns. You must have a copy of these forms as they are not unique, they are forms that already exist. Secondly, the questions that are sent to a claimant when they report a change are already set, again they are not unique, there must be a copy of all the available questions otherwise a case manager wouldn’t be able to send them. In this interest of transparency please send a copy of all the forms UC have and also all the questions that can be sent.

Yours faithfully,

Craig Adams
