Type II Diabetes Education Programmes
Dear Bassetlaw Primary Care Trust (PCT),
I am writing to request information under the Freedom of
Information Act 2000. I am outlining my query as specifically as possible.
I would like to understand the Trust’s provision of Type II Diabetes Education Programmes, specifically those commissioned through your GP practices and consortia. Where possible, I would be interested to know the number of current and previous patients supported, the name of the chosen education programme(s), and the number of surgeries involved.
I am interested in any information held by you regarding my
request. I understand that under the Act, I do not have to specify particular files or documents and that it is your department's responsibility to provide the information I require. If you need further clarification, please contact me by email.
I would like to receive the information as electronic copy, via WhatDoTheyKnow.com.
If my request is denied in whole or part, I ask that you cite one of the Freedom of Information Act exemptions for each piece of information you refuse to release. I would also expect you to release all non-exempt material. I reserve the right to appeal your decision to withhold any information or to charge excessive fees.
I should be entitled to a response within 20 working days. I would be grateful if you could confirm in writing that you have received this request. I look forward to hearing from you.
Yours sincerely,
Kate Walker

Answer
Within Bassetlaw a structured education for patients with diabetes is
commissioned by NHS Bassetlaw, open to patients registered with all
practices in Bassetlaw and was provided by Bassetlaw Community Health.
The DESMOND programme is provided and from April 2010 there have been a
total of 269 attendances.
I trust that this fulfils your request.
Kind regards
Sarah
