Type II Diabetes Education Programmes

Dear Great Yarmouth and Waveney Primary Care Trust (PCT),

I am writing to request information under the Freedom of
Information Act 2000. I am outlining my query as specifically as possible.

I would like to understand the Trust’s provision of Type II Diabetes Education Programmes, specifically those commissioned through your GP practices and consortia. Where possible, I would be interested to know the number of current and previous patients supported, the name of the chosen education programme(s), and the number of surgeries involved.

I am interested in any information held by you regarding my
request. I understand that under the Act, I do not have to specify particular files or documents and that it is your department's responsibility to provide the information I require. If you need further clarification, please contact me by email.

I would like to receive the information as electronic copy, via WhatDoTheyKnow.com.

If my request is denied in whole or part, I ask that you cite one of the Freedom of Information Act exemptions for each piece of information you refuse to release. I would also expect you to release all non-exempt material. I reserve the right to appeal your decision to withhold any information or to charge excessive fees.

Dear Ms Walker,
 

In response to your FOI regarding Type II Diabetes Education Programmes

 

DESMOND is currently commissioned via a single provider, since the
commissioning of DESMOND in January 2009, 998 people have been referred of
which 614 have completed the programme. All 27 GP practices are able to
refer a newly diagnosed type II diabetes individual to DESMOND.
