Dear NHS Mid and South Essex Integrated Care Board,
The CCG/ICB now says TMS is routinely funded and no longer requires an IFR request to fund TMS on the NHS
Historically over the years, Mid Essex CCG did not fund ANY ifr for tms but the partner ccg Basildon and Brentwood funded all 9 requests source ccg own data May 2020.
So what TMS treatment protocols does ICB the latest incarnation of the NHS accounts department routinely fund?
Left DLPFC?
Right DLPFC?
OPC?
SMA?
DMPFC?

This FOI request does relate to TMS
I wanted to bring to your attention some total contradictions in FOI responses I have recently received from the ccg and ics.
in a previous FOI Request: TMS requests and number funded Date: 22nd June 2022
Our Reference Number: 2223049 Mid Essex CCG
"To confirm TMS is now funded for by all Mid and South Essex CCGs. This has been the case from April
2022. Please accept our apology for all the misunderstandings and shortfalls."
However the above is totally contradicted rTMS/TMS is now claimed by the ics to not be a routine treatment your response of 22nd September 2022 Our Reference Number: 2223139
"Exceptionality needs to be demonstrated in order for us to fund rTMS as this is not something that is
currently commissioned"
The new ICS has yet to answer the FOI request questions that you asked for clarification on the abbreviated terms and explained earlier what they mean today regarding TMS treatment.

In reply to your query, please see below:
• We fund RTM as an offer which is part of complex care and it is in
development to include the wider pathway. Or pathway of offer is not
split on specific targets as listed but rather a whole offer which it
is dependent on the clinical team to determine the individual
requirement.
• EPUT is the only contracted provider as per our commissioning
agreement for MH offer locally and as thus the routine provider for MH
based on the determination of clinical need.

you have given an answer that provides little detail and is none committal on what forms
of TMS the ics will fund.

Still waiting to hear if you are going to respond to my review questions of 01-12-2022?
The question is logical to me, as the ICS would not fund all physical operations eg cosmetic surgery.
Therefore it would make sense for the ICS not fund all possible MS protocols and therefore should be able to answer yes or no to the tms protocols in my request for review again onthe 01-12-2022.

Maybe the ICB only funds TMS treatments that are NICE approved on the complex care pathway?
That would then define what TMS treatments the ICB will routinely fund and answer my question
if the above is the answer it's a shame the ICB has not stated this before.
also, after discussion with the ICB and myself, no mention was made of NICE approval defining what TMS treatments the ICB wIll routinely fund
All that was mentioned in this discussion was the responsible clinician's recommendation was needed for any TMS treatment not that it had also to be NICE approved.
If what I have stated above is true why does the ICB not communicate clearly, rather than send patients off on a wild goose chase?

However, my latest reply of the 08/02/2023 is not true. As on the complex care pathway, the commissioning bond funds at the Eput Neuromodulation treatments that are not NICE recommended in England for mental health including
1, Esketamine
2, Vagus nerve stimulation
Therefore my recent comment on what the body will fund having to be NICE-approved is not true on the complex care pathway.
So what TMS treatments are funded from my list on the complex care pathway, has the body not set any restrictions?
