Swansea Social Services: Aaron Gilbert – SCR omitted information

Dear Swansea Council,

No record appears in the published Serious Case Review [SCR]/summary into the death of Aaron Gilbert of failures having been identified within the Service prior to his death, for example the CSSIW [inspectorate] visited and investigated in February 2005 regarding Laming Report [2003] failures and procedural failures [ACPC] identified in the service in a private law case in Swansea and warnings of a child death being raised.

Please provide any information on the policy, guidance or other process on the withholding of information from the SCR investigator or any other enquiry agency which would be useful to the inquiry or investigation in identifying failures in social work practice and child protection.

Please provide any information in electronic format.

if there is no policy on withholding information why is this pertinent information not in the Serious Case Review? Did the inspector appointed from the NSPCC withhold it?
