8. A list of the names of providers who have been awarded supported living placements off the the framework and the number of placements each of the providers has won. Please provide split between below £700 per week and above £700 per week
9. The number of placements made through the framework agreement that have broken down (ie the care provider has had to be changed or the person has had to move to another premises including hospitals). Please provide split between below £700 per week and above £700 per week
9. A copy of the budgeting model, together with its underlying assumptions used to calculate the budget for each supported living framework mini-tender referral.

Point 1 - 14 and 24 mini tenders but at point 2 - 15 and 23 mini tender - does not agree should be the same so which is correct

at points 4 and 5 (less that £700) 3 successful and 7 unsuccessful - but 14 or 15 mini tenders - what happened to other 4-5 mini tenders?

at points 4 and 5 ( greater than £700) 11 successful and 17 unsuccesful - giving total of 28 mini tenders. but you say there were only 23 or 24 mini tenders at points 1 and 2.

Dear Mr Chang,

Thank you for your e-mail of 2 June 2015.

Please be advised that most were not at a stage where a decision had been
made to choose a provider and source a property. They are still open
referrals and we request periodic updates from the allocated social
workers.
 
Other projects were not progressing for reasons including:
 
a) Family had changed their mind and wanted to continue support at home.
b) Residential Care deemed more appropriate
c) Customer detained in hospital
d) No provider interest

I hope this helps to clarify this for you, but please do not hesitate in
contacting me if you have any further queries.
