Supply copies of certain PPE Requisition Documents

Dear Rotherham Metropolitan Borough Council (RMBC),

Please provide scanned copies of all Personal Protective Equipment (PPE) Requisition Forms issued to Rotherham’s COVID stores (the COVID stores) relating to distributions to each recipient domiciliary care organisation registered with the Care Quality Commission (CQC) during the period 1st March 2020 to 1st June 2020 inclusive.

These forms:
(a) were issued by the COVID stores/ RMBC with various collection dates.
(b) contain details of the recipient organisation as well as items and quantities of PPE stated as being distributed to / and collected by each of them on any particular date.

A single document containing all the requested scans in any logical order is acceptable. The organisation, amount(s) and the date should, in particular, be clearly visible.

Further to your email of 14th September the very straightforward request I made for certain copy documents has now clearly not been processed by you within a reasonable time as a further 14 days has now passed beyond the 20 day statutory threshold. Please respond without further delay.
