Dear London Strategic Health Authority,

Under the Freedom of Information Act, we request
information relating to Serious Untoward Incidences (SUI);

1.The number of serious untoward incidents (SUIs) reported by year since the 2006 calendar year;

2. Original full summaries of each serious untoward incident provided by reporting NHS bodies.

3. Brief SHA summaries in laymans terms or with anonymised data.

4. The number of foi requests received for sui data since the 2006 calendar year.

We would prefer the request in electronic format where possible.

Dear Elsa,

Thank you for your request. NHS London is in the process of considering whether the information you have requested can be provided and you should receive a response by 19th December 2012.

I wanted to take this opportunity to advise you upfront that should this be possible, and FOI exemptions did not apply to some or all of the information, NHS London would not be in a position to provide this information within 20 working days. This is because your request covers such a vast amount of information potentially needing to be redacted, that realistically this would take a number of months to complete. I will keep you updated on the progress of your request and will discuss any practicalities if and when they arise.
