Please could you provide the following information.

.How have you obtained the views of parents/carers on the reforms? Please provide evidence of the methods used.

.How have you recruited parents to trial EHC plans? For example, were families chosen by the Council or did you ask for volunteers? Please provide evidence.

.Please provide copies of the information provided to parents relating to the EHC plan pilot.

.What selection criteria were used to recruit pilot families to test EHC plans? Please provide a copy of the selection criteria.

.How many children presently have EHC Plans in your LA?

.How many of the children with EHC Plans also have statements of SEN?

.Have any of the families who are trialling the EHC plans withdrawn from the pilot? If so, how many and what are the reasons for their withdrawal?

.How have satisfaction levels of pilot families testing the EHC plans been measured and please provide data showing responses?

.How many children have Direct Payments for SEN under the Special Educational Needs (Direct Payments) (Pilot Scheme) Order 2011 (SI 2012/206)? Please exclude those direct payments made for travel or social care which could have been provided without the Pathfinder pilot.
