Dear Birmingham City Council, on 17/10/13 you began an Assessment on my child under CF-ID 60699059 After receiving a children social care inter referral application From Cafcass, this was marked as No further action.

On 01/10/13 Cafcass sent a letter to FAO Duty Manager at IAT City Wide PO Box 16635, the letter asked the council to make further enquiries with the family and child, This lead to you commencing the Assessment mentioned above under a child assessment.

The Form used for the Assessment was a Single Assessment dated June 2013, pages 1-10 on my form there was section 11, Managers Analysis and Decision, on the Assessment this part was not filed in? please explain why this was not filed in?

Please send me a copy of the complete form, showing all pages as there should be a box where signatures are used for those who undertook the assessment?

Please send me the Link where this form is on the council website.

When a inter referral is received from another agency such as Cafcass , is the correct form to use a CAF Common Assessment Frame work? not a Single Assessment? for a child under 6 year of age? if there is previous history known?

When completing this form is it only the council who take part or do Cafcass also share there views on the form? as this is report by the local authority not Cafcass?

If the parents are in a private law case in a family court and Cafcass are involved why would the Council then become involved in a private law case? would this not be the job of Cafcass and the Judge to order such a report if needed?

When the Social worker updates the client activities was this back in 2013 done on CA V1.0.1 January 2008 form? if so please share the link to this form and if not please send the correct form used in 2013. given this was 5 years out of date being 2008. please provide the form for 2007 as well.

Please supply the IAR1 form dated August 2009 that cafcass would of used to make a referral, is this the correct form as of 2013 that was being used by Birmingham City Council?

Please supply the form used for client case records in 2013 the form i have is CCR-V1.0.0 October 2006 but this would be 7 years out of date ?

Please explain why my case was changed from Ms Jane Daly to Ms Angela Blake in October 2013 what was the need to have new Social worker appointed when Ms Daly had began the assessment by making calls in October 2013?

The report state the School were spoken to on 19/09/13 but my child school have no record of it being this date? and the referral from Cafcass was not made until 17.55 on 19/09/13 so the the council would not had sight of this until 20/09/13?

I also called the council on 19/09/13 but there is no record of this call on the records provided by the council ? why is this missing or been removed? I spoke to a lady named Angela, would this be Ms Angela Atkinson on the 19/09/13?

When the Assessment was carried out the Social worker did not speak to me until 09/12/13 this is proof on my phone records, as i had to call the 0121 number for her mobile number, on 03/12/13 i was then ignored until 09/12/13 by Ms Angela Blake this was after the Assessment was completed i was made aware of later on? why was i not spoken to by a social worker until 2 months after the Assessment began?

Cafcass also requested data from the council in November 2012 and August 2013 but the form for 2012 is blank and appear not to of been completed?

Also the information shared by Ms Lisa Hartwell failed to share all the date reported on 07/09/12 by my self and is reported as being shared on 14/09/12 why is the date incorrect?

would this not be an act of fraud to conceal data from reports and issue of DV reported by other in September 2013? concealment under the Fraud Act 2006.
