Please provide details of your organisation's sickness absence both
for 2009-10 and 2010-11

In particular, I am interested to see the above requested
information in the following breakdown:

1. Total days lost to sickness absence in relation to total
available days for work for the whole year, both in business year
2009-10 and 2010-11
2. Total percentage of sickness absence rate against contractual
time, both in business year 2009-10 and 2010-11
3. Total cost of sickness absence both in business year 2009-10 and
2010-11
4. Percentage of sickness absence rate in each directorate, both in
business year 2009-10 and 2010-11 and cost to sickness absence in
each directorate
5. Provide five most common reasons for sickness both in business
year 2009-10 and 2010-11 and associated total cost for each reason
6. Provide the name of top three departments where the cost to
sickness absence attributable to Stress/Anxiety or other Mental
Health problems were the highest, both in business year 2009-10 and
2010-11
7. Number of short-term sickness absence due to Stress/Anxiety or
other Mental Health issues with associated cost broken down to each
directorate, both in business year 2009-10 and 2010-11
8. Number of long-term sickness absence due to Stress/Anxiety or
other Mental Health issues with associated cost broken down to each
directorate, both in business year 2009-10 and 2010-11
9. Does your organisation provide any training to managers on
managing Sickness absence in particular managing mental health
related sickness absence? Is this training compulsory for managers?
10. Please provide attendance details of any compulsory training
you provided on Managing Sickness Absence in particular on Managing
Mental Health related absence. I am only interested in total number
of managers with line managerial responsibilities and the total
number of managers with line managerial responsibilities attended
training course on Managing Sickness Absence in particular managing
Mental Health related absence
