I am writing on behalf of David Davies to respond to your information
request.
 
 
1)      What evidence do you have that the statement is accessible to
families (i.e. unique visitors/ postal reach etc.)?
 
The statement has previously been published on the borough website. Since
September 2014 it is published on the RBWM Local Offer, including a record
about Short Break Statement. We have evidence that is has been accessed
from the Local Offer website . If required in alternative formats, we can
make this available.
 
2)       How often do you review the Short breaks statement? When was the
last time you revised it and for what reason?
 
Short Break Statement was first written and published in 2011. It was
refreshed in 2013 to take account of service changes. It is due for
revision in 2016.
 
3)      How many children have been refused short breaks services because
of eligibility criteria in the last twelve months? How many of these had
an assessment?
None.
 
4)      How often do you consult families on the content of the short
breaks statement? When did this last happen and how many families were
involved?
 
At last refresh, questionnaires were sent to all 320 children on the LDD
caseload either to complete online or in hard copy.  The consultation
period ran from 10th November to 7th December 2012.
The questionnaire was also sent to all families known to the Parent
Partnership Service so that those who may be eligible to receive LDD
Services, but are not currently accessing them, would be aware of the
short break offer and how to access it.
 
The consultation included a focus group discussion for parents and carers
held at Saturday Club on 10th November, which was attended by 9
parents/carers. This will feed in to our Short Break statement.
 
Comments about the short break services from families received through
other consultation mechanisms were also included in the refresh.
Consultation with children and young people is ongoing about short breaks.
 
5)      What evidence do you have that you deliver short breaks to
children and young people as outlined in your statement?
 
We monitor the take up of places and use of provision. We commission
places from external parties and monitor usage.
 
6)      How many children do you have records of that you consider to be
currently eligible for short breaks?
 
Approximately 300 children open to Children & Young People Disability
Service are eligible for short breaks.
