Dear Derby Hospitals NHS Foundation Trust,

Please may I have the following information and documents:

1 The current procedure for operation of the Derby Hospital helipad.

2 The issue dates and withdrawal dates of any previous versions of procedures for operation of the hospital helipad.

3 Copies of any previous procedures for operation of the hospital helipad.

4 A copy of the audit schedule for the hospital management system.

5 Copies of any audits or review of the hospital helipad or related procedures.

6 Copies of any reports, communications, or meeting minutes relating to the operation of the hospital helipad.

Please could you clarify what you mean when you use the term "hospital management system"?

Dear Mr Moore,

To summarise our telephone conversation, by the phrase "hospital management system", I meant the hospital quality management system. My working assumption was that the hospital would have a routine audit programme for the internal audit of its quality management procedures, and it was this to which I referred.

I have a concern about the validity of my working assumption that the operation of the helipad was defined by the procedure 'Helipad Operational Policy'. There is a significant difference between the helipad log and what'Helipad Operational Procedure' requires in Appendix 2.

If there were two separate procedudures in effect, I would be happy to rephrase my questions in a more appropriate manner.
