I would like to request a copy of your organisations register (or registers) of gifts and hospitality for Senior Staff as named below:

Specifically I would like to obtain a complete record of all the gifts and hospitality offered to, or accepted by, the above named officials.
Please provide electronically, a copy of this information for 2009, 2010, and up to the present date. For those staff named above who were not in post during these years, please provide the information for their predecessors.
I would be grateful if you could confirm in writing that you have received this request, and I look forward to your response within the statutory time limit of 20 working days.
