Record of a reportable accident (RIDDOR) at Springhead Brewery

The request was refused by Health and Safety Executive.

Dear Health and Safety Executive,

Can you please tell me if a reportable incident was reported to you on a site owned by Springhead Fine Ales Ltd.

I would like to know if a brewing vessel explosion that caused RIDDOR reportable injuries to 2 employees was ever reported to HSE and if any action by HSE was ever undertaken.

A brewing vessel door burst open injuring 2 members of staff, namely Keir Teasdale who was airlifted and hospitalised for 2 weeks.

The brewing vessel door has two bolts. One bolt had been unusable for a number of months, so only one bolt was securing the door, under brewing pressure. Until the day that single bolt failed and the contents badly scalded 2 members of staff who happened to be standing adjacent to the vessel at that time. Keir Teasdale, being the son of the owners or the company, Helen Teasdale and Richard Scragg.

I can't remember when the accident happened, I think it might have been in 2013, 2014 or 2015.
