Record Keeping Audit
Dear Bolton NHS Foundation Trust,
1. Your Health-Record Keeping Standards policy (2012 update), states that there will be quarterly reviews of compliance record keeping standards.
2. Quarterly record keeping audit reports are referred to in the CQC inspection report dated May 2012 which states “We saw the trust-wide results of the record keeping audit for the period January to March 2012 ... Audit results seen for the period November 2011 to January 2012 from the divisions within the hospital indicated that ...”
3. Please can you provide copies of thee quarterly record keeping audit reports from January 2012 to December 2015.

Please note the time the attached Record Keeping Audits were produced the
Trust had paper based clinical notes. The Trust now uses electronic
patients records.
