Dear Parliamentary and Health Service Ombudsman,

In March 2016 following an FOI I was informed that;

However, in line with our duty to provide advice and assistance under
section 16 FOIA, I can confirm the five quality assurance associate posts
were part of a wider associate caseworker recruitment campaign which took
place in October 2015. Further details about that recruitment campaign can
be found at:
[2]https://www.whatdotheyknow.com/request/a....

The quality assurance associates are part of the Quality Service and
Integrity department and they report to the quality assurance manager. The
associates are self-employed and I have attached the job description
relating to this role for your information.

https://www.whatdotheyknow.com/request/e...

The 5 'external' quality associates were employed to randomly audit a minimum of 350 cases per year to examine the quality of case investigations following criticism from the Patients Association and the Health Select Committee.

1. Can you tell me whether there are any quality assurance associates still operating within PHSO?

2. How many are still operating with PHSO?

3. How many cases were randomly reviewed in 2017/18?

4. If no quality associates are still employed at PHSO can you tell me when the decision was made to cease their employment?

Yours faithfully,

Della Reynolds
