Dear Mr Ogden

I am writing to confirm that the Humber NHS Foundation Trust has now
completed its search for the information which you requested on 21 May
2010.

You asked for:

1) The total number of public health funerals (where the deceased
has no relatives or there is no one else willing to pay) that your Trust
has undertaken for the financial years between 2007 and 2010

2) The number of public health funerals that your Trust has
undertaken by gender for the past 3 financial years

3) The number of public health funerals that your authority has
undertaken by age for the past 3 financial years

4) Since April 2009, how many cases have there been where family and
friends refuse to finance or make arrangements for their relatives
funeral?

5) How much have the burials/cremations cost the trust broken down
by year for the last three financial years?

6) How much does your trust typically (average cost) pay for a
Public Health Funeral?

7) What does your Trust provide at a Public Health Funeral?

8) What relationship do you have with your local council(s)
regarding funerals of this nature?

This information is not routinely kept by the Humber NHS Foundation Trust.
To our knowledge the Trust has only dealt with one funeral which meets the
criteria you have mentioned. This was for an elderly female patient. We do
not have her exact age. The funeral was paid for by the local authority
under the "public health funeral" fund. The Trust was not asked to pay
towards it. The funeral was taken care of by the Coroner once we confirmed
we had no knowledge of next of kin.

I hope my response has covered all the information which you requested.

To help us measure our performance in responding to Freedom of Information
requests I wonder if you will take a little time to complete the attached
form and return it to me at this e mail address.

Foundation Trust and provided to you free of charge for your personal use

Regulations on the Re-use of Public Sector Information Regulations 2005,
you must ask us for permission to do so in respect of each specific piece
of such information. If we do grant such permission this may involve a
licensing arrangement which may attract a fee.
