Pages 50/51 of your October 2019 board papers cover a project with Telefonica (who are normally known in the UK as O2)

Can you please provide relating to this project:

1) Any data sharing agreement, contract, or memorandum of understanding between the Trust and Telefonica/O2, whether for this project alone, or for overarching projects involving data sharing. If you have separate documents for different phases, provide both;

2) Any Information Governance assessment, Data Security and Protection Toolkit assessment, or Data Protection Impact Assessment done for the project and held by the Trust (whether produced by Telefonica/O2 or the Trust), covering phase 1, phase 2, or both;

3) The final report of phase 1 of the project;

4) The material provided to service users about the mobile network data aspects of phase 2 as part of what your board papers describe as "Extensive information governance and communications preparatory work is being undertaken alongside active engagement with service users";

5) any lines to take from your communications team relating to the recent HSJ piece, including and especially the choice (whether yours or otherwise) to brand this project as "Telefonica" compared to the usual company branding of "O2";

Should you choose to determine that any part of this would breach the cost limits, please detail how you believe that to be the case, and respond to the other parts of this request.
